EHR Integration Activation Guide for SNFs

Step-by-step instructions for Skilled Nursing Facilities to activate Sparkco integrations. Follow these guides to enable FHIR API access with your PointClickCare system and help your hospital partners connect through Epic.

Part 1

PointClickCare USCDI Connector Activation

For SNF Administrators: Send the email template below to PointClickCare to enable Sparkco integration with your facility's EHR system.

Activation Process

SNF Initiates Request

After signing up with Sparkco, send an email to USCDIConnector@pointclickcare.com using the template below. Make sure to include your facility's legal name and all locations where you want the integration enabled.

Important: PointClickCare requires all binding transactions to contain official legal names. Ensure your customer uses their legal organization name as registered with the Secretary of State.

Quotation Process

The email triggers a quotation process between PointClickCare sales rep and the customer. Once complete, an internal PCC enablement case will be created.

Activation Confirmation

After the enablement case is actioned, you'll receive an email confirming app activation. To retrieve the tenantID needed for FHIR API calls, refer to section 1.1.2 in the documentation.

Timeline: Minimum 2 weeks for data availability. Once integration is complete, your EHR launched application will be visible to users.

I understand that a PointClickCare Account Development Representative will be contacting me with a Quote/Order Form and associated agreement which must be signed prior to going live.

The Quote/Order Form and associated agreement authorizing PointClickCare to share PHI with third-parties (the USCDI Connector App vendor) requires the full legal name of my organization (the official name as registered with the Secretary of State or Secretary of Commonwealth in the US) and must be signed by an authorized representative at my organization who has authority to bind the company.

I understand that I must contact my PointClickCare Customer Success Manager to request application disablement. I will need to provide the Vendor AppName, Customer Legal Organization Name, and List of Facilities where application is to be disabled.

Epic FHIR Application Activation (For Hospital Partners)

For SNF Administrators: Share these instructions with your hospital referral partners who use Epic. They will need to request activation to send referrals to your facility through Sparkco.

Hospital Sends Activation Request

Your hospital partner should email VendorServices@epic.com with the application details and Connection Hub request. They can use the template below to ensure all required information is included.

Connection Hub Review

Epic will review your application for Connection Hub listing. This directory helps healthcare organizations discover and connect with verified applications.

Production Activation

Once approved, your Client IDs will be activated for production use. Healthcare organizations can then enable your application in their Epic environments.
